Once your appointment is scheduled make sure you follow the steps for your patient portal and fill out all necessary forms. If you didn’t receive an email invite please call the office. Forms must be completed 2 days before your appointment or your appointment might be rescheduled.
Please arrive 5-10 early. If your more than 5 minutes late, you might be asked to reschedule.
